Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a passionate Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN) to join our team at Genesis Health. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans to meet Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• LPN in good standing with current state licensure.
• Prior experience in healthcare settings is required.
• Proficiency in RAI/PPS processes.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
• BLS/CPR certificate required.
